Levaquin (levofloxacin) Dose Optimization

 We will convert all Levaquin orders to the 750 mg daily dose or the equivalent renally adjusted 750 mg regimen.  Please follow the below process for adjusting these orders. 

For new orders, all patients (regardless of indication) will receive an initial 750 mg dose followed by subsequent doses per the patient's renal function.

Also:

- send CI to alert physician of the change

- edit label comments for the Levaquin entry that will outline the auto adjustment.
