Lovenox Therapeutic (Treatment) Dosing
THEARAPEUTIC (TREATMENT) Dosing:
- For est.CrCl >/= 30
ml/min: 1 mg/kg SubQ every 12 hours
- For est.Cr.Cl < 30
ml/min: 1 mg SubQ once daily.
- If Cr.Cl <20
ml/min, will order anti-Xa level with third dose
OR if pt. morbidly obese, or producing NO urine, will call physician to
CHANGE TO HEPARIN DRIP. CALL- DO NOT LEAVE NOTE!!!
If you are ordering an anti-xA level to monitor the safety of enoxaparin
in patient's with CrCl < 20 ml/min:
- When to order: 4 hrs after the 3rd dose of enoxaparin.
- Therapeutic range: 0.5-1 units/ml. Please note that this therapeutic
range is for twice daily treatment dosing (1 mg/kg) ONLY.
