heparin should generally not be interrupted during procedures unless there is a specific physician order.

 • Two RNs independently verify dose, pump settings, and that patient is not on another anticoagulant prior to administration.
 • Weight-based dosing and monitoring protocol and policy
 • Pharmacists follow patients daily
 • Located in separate compartment in dispensing cabinet from Lovenox.
Per policy, these medications require cardiac and/or special monitoring.
