P & T Update Memo June 2017

Following is a P and T Committee update (from the June 27th meeting). Starting date for specific programs is 3 July 2017, unless otherwise noted. Please let me know if you have any questions or comments. Don’t forget to check the “New Drug Monitoring Spotlight” section of the website, which provides important monitoring information about newly approved drugs. Also, attached is the updated version of the “Not Stocked, Not Ordered” drug list with the new additions highlighted in red.

Dextroamphetamine has very low use for inpatients. Decrease waste by only stocking the 5 mg ER product and using it for other ER strengths and IR in same daily dose.
imipramine pamoate (Tofranil-PM) Update

Non-Formulary


Automatic Interchange Approved

Orders for Tofranil PM will be interchanged to imipramine HCl at an equivalent dosage administered multiple times per day.
tenofovir alafenamide (Vemlidy) Formulary, Not Routinely Stocked Addition of this product to the formulary allows for combination products Descovy and Odefsey to be reclassified as non-formulary and dispensed as the individual agents.
crisaborole (Eucrisa) topical Non-Formulary

Inpatients may use home supply.

Other formulary treatment options available: topical emollients, topical steroids (formulary doc), and tacrolimus (Protopic).

nitric oxide (INOmax) inhalation Formulary, Restricted

Restricted to use by neonatal & pediatric critical care, cardiology, pulmonology, and trauma.

Use in adults pending update in respiratory therapy policies & procedures.

nitrous oxide inhalation Formulary, Restricted

Nitrous oxide continues to be utilized in the surgical areas. 

Approved use was expanded to include procedural sedation applications in the ED.

Use in adult ED areas pending update in respiratory therapy policies/procedures.

ocrelizumab (Ocrevus) Formulary, Restricted Restricted to use in the outpatient setting by neurology.
Quinolone / Steroid Otic Solution Combination Update

CiproDex – Reclassified as Formulary

Otovel has an advantage of being available in unit-dose amps for single doses. Ciprodex bulk bottle will be utilized when more than one dose is required.
