P & T Update Memo March 2016

Following is a P and T Committee update (from the March 22nd meeting). Starting date for specific programs is 28 March 2016, unless otherwise noted. Please let me know if you have any questions or comments. Don’t forget to check the “New Drug Monitoring Spotlight” section of the website, which provides important monitoring information about newly approved drugs. Also, attached is the updated version of the “Not Stocked, Not Ordered” drug list with the new additions highlighted in red.

albuterol Oral Formulary Update

Vospire ER and other albuterol oral long-acting agents – Non-formulary, not stocked.


Automatic interchange approved.

Interchange albuterol oral agents to same daily dose of albuterol IR 2 mg in divided doses.

Albuterol IR 2 mg x 2 will also be used for the 4 mg dose.

captopril-HCTZ (Capozide) Update

Reclassifed as non-formulary, not stocked.


Automatic interchange approved.

Dispense each agent separately.


Doses of 15 mg of HCTZ will be converted to 12.5 mg.

nitroglycerin ER Capsules

Reclassified as non-formulary, not stocked.


Automatic interchange approved.

Interchange to isosorbide mononitrate SR.
bendamustine (Bendeka)

Reformulation


Formulary, restricted to Oncology.

Bendeka is a new formulation of bendamustine, replacing Treanda, administered in a lower-volume and shorter infusion.
asfotase alfa (Strensiq) Non-formulary, not stocked. Patients may use home supply
clonidine XR (Kapvay) Update

Non-formulary, not stocked.


Approved automatic interchange remains for adults only.

In pediatrics, contact MD to discuss use of IR clonidine or alternatives.

If a pediatric patient is converted to IR, increased monitoring is recommended Patients may use home supply.

Antibiogram 2015 Informational.

Incidence of most multi-drug resistant organisms decreased slightly in 2015.

The updated antibiogram will be published in the 2016 ORG.

ofloxacin Otic Drops (Floxin) Shortage Automatic interchange approved.

Interchange to ciprofloxacin 0.3% ophthalmic drops on a 1:1 dosage ratio and same dosing interval during shortage.

Labeling reminders to avoid route confusion to be implemented.

Vitamin D2 Liquid

Formulary, restricted to no substitution in pediatric patients.


Automatic interchange approved.

Vitamin D2 (ergocalciferol) 8,000 units/mL will be stocked at HHWC.


Orders without NO SUB will be dispensed as Vitamin D3 (cholecalciferol) 400 units/mL
1 mg:1 mg

Formulary Policy Update Updated policy revisions approved.

Medications are designated as formulary unrestricted, formulary restricted, or non-formulary.
