P & T Update Memo September 2016

Following is a P and T Committee update (from the September 27th meeting). Starting date for specific programs is 3 Oct 2016, unless otherwise noted. Please let me know if you have any questions or comments. Don’t forget to check the “New Drug Monitoring Spotlight” section of the website, which provides important monitoring information about newly approved drugs. Also, attached is the updated version of the “Not Stocked, Not Ordered” drug list with the new additions highlighted in red.

acetylcysteine (Cetylev) Non-Formulary. Alternative formulations for intravenous and oral routes are currently on formulary.
sofosbuvir-velpatasvir (Epclusa) Non-Formulary. Inpatients may use home supply.
pimavanserin (Nuplazid) Formulary, Restricted.

Initiation of therapy is restricted to psychiatrists or neurologists.

Any prescriber may continue maintenance therapy upon admission to the hospital.

Fusion Plus – PreNatal Non-Formulary, Therapeutic Interchange Approved. Interchange to Prenatal Plus, the formulary workhorse for prenatal vitamins.
oxycodone ER (Xtampza ER) Non-Formulary, Therapeutic Interchange Approved. Interchange to oxycodone CR (Oxycontin) with a dosing ratio of Xtampza ER 9 mg: Oxycontin 10 mg.
ferric pyrophosphate citrate (Triferic) Non-formulary.

An iron product administered via hemodialysate was determined not to be needed at this time.

Intravenous iron preparations are currently on formulary for iron repletion.

intranasal influenza vaccine (FluMist) Non-Formulary for 2016-2017 influenza season. CDC recommends against use for the 2016-2017 influenza season.
Oralair (Oral Allergen Extracts) Non-Formulary. Inpatients may use home supply.
fentanyl patch waste deactivation system (Deterra) Informational. Use of the deactivation system went into effect September 12, 2016.
Duplicate Medication Orders and PRN Indications Policy Update Policy Revisions Approved.

Policy updated and expanded to include additional indications (insomnia, muscle spasm, cough, BP) eligible for automatic pharmacist discontinuation when duplications exist.

The policy was also expanded to include guidance for automatic addition of PRN indications by a pharmacist for select medications when no indication is specified by the prescriber.
