P & T Update Memo December 2016

Following is a P and T Committee update (from the December 13th meeting). Starting date for specific programs is 19 Dec 2016, unless otherwise noted. Please let me know if you have any questions or comments. Don’t forget to check the “New Drug Monitoring Spotlight” section of the website, which provides important monitoring information about newly approved drugs. Also, attached is the updated version of the “Not Stocked, Not Ordered” drug list with the new additions highlighted in red.

melphalan IV (Evomela) Non-Formulary
Automatic Interchange Approved
Continue to use melphalan IV (Alkeran). Evomela has longer stability than older product but is much more costly.
nitroglycerin SL powder (GoNitro) Non-Formulary
Automatic Interchange Approved
Interchange to sublingual nitroglycerin at same dosage.
phentolamine (Regitine) shortage Formulary, Restricted to use for treatment of hypertension crisis situations where increased catecholamine release is involved (i.e., pheochromocytoma) Only a few vials currently on hand, unknown how long shortage will continue, other main use is sympathomimetic extravasation and terbutaline can be used for these cases.
lifitegrast ophthalmic solution (Xiidra) Formulary Available in unit dose amps (60 quantity). Xiidra has a different mechanism than Restasis, and does not start working for about 14 days.
obeticholic acid (Ocaliva) Non-Formulary Specialty pharmacy only sends directly to patient. Inpatients must use own supply, rare use expected.
short ragweed pollen extract (Ragwitek) Non-Formulary Initiation should be deferred until after discharge. Inpatients may use own supply.
Implementation of Adult Hypoglycemia Orders Automatic Pharmacist Initiation Approved Pharmacists may automatically implement the Adult Hypoglycemia Orders for patients with active orders for scheduled or PRN insulin.
Infusion Reaction/ Anaphylaxis Order Set – Outpatient Medical Approved For general use on drug infusion orders in OPM Department. Pending further review from institutional committees.
