Pulling and Checking Batched Items PSP has had multiple mistakes in the last few weeks when sending previously batched items. Precedex was sent labeled with iCare insulin label and insulin sent with iCare precedex label. Lasix 200 mg/100 mL has been sent as 80 mg/100 mL to CHF and precedex pulled for sodium phosphate. ALWAYS double check to make sure the iCare label matches the PK label when pulling from the refrigerator and checking. |
IVIG Backorder We are out of stock of Gammagard. We are unable to order Flebogamma at this time. Gamunex supply is getting short too, be on the look out for more info soon. |
Q faxes to 52645 When receiving faxes in PSP that need to be routed to the Q (such as exparel for CVOR) please send through HCS at 52645 rather than the hard copy fax number. |

Vanco Placeholder Orders When following ESRD patients, PKS pharmacists may use a placeholder order so the physician is aware the patient being followed. The comments on these orders will say “placeholder order – DO NOT GIVE”. If you see this throw the label away - it is not intended to be given. |
¼ NS OK to make We received supply of 30 mL vials of sodium chloride and are ok to resume making ¼ NS. We have about 3 month supply of 200 mL bulk bottles for TPN compounding. |
EPINEPHrine 8 mg /250 NS for CVOR Starting Monday 8/13 midnights will begin preparing EPINEPHrine 8 mg/250 mL NS for CVOR. CVOR and CVICU will change to using 8 mg/250 mL for their standard concentration on Tuesday 8/14. |
Iron Dextran and Amicar Back in Stock Amicar is back in stock, PSP can now resume making aminocaproic acid drips for the OR. CVOR will continue to use tranexamic acid 2g/500 mL for their patients. |
Fasenra for OP Medical benralizumab (Fasenra) is in stock for OPM. It is similar to Xolair and Nucala, but comes as a prefilled syringe. It is on the PSP carousel, so pick from the carousel when removing from stock. |

Dilaudid Syringes Back in Stock We currently have supply of HYDROmorphone 1 mg/1 mL syringes/tubexes/vials, so at this time PSP is no longer drawing them up. The tubexes do have to be filtered by the nurse prior to administering to the patient (see pulse page announcement for more info). |
