P & T Update Memo June 2018

Following is a P and T Committee update (from the June 26th meeting). Starting date for specific programs is 1 July 2018, unless otherwise noted. Please let me know if you have any questions or comments. Don’t forget to check the “New Drug Monitoring Spotlight” section of the website, which provides important monitoring information about newly approved drugs. Also, attached is the updated version of the “Not Stocked, Not Ordered” drug list with the new additions highlighted in red.

emicizumab-hxwh (Hemlibra) Formulary, Restricted

Restricted to use in outpatients settings ordered by HEM/ONC specialists for prophylaxis of bleeding in patients with Hemophilia A (congential factor VIII deficiency) with or without factor VIII inhibitors. Inpatients may use own supply.

If inpatient’s supply is unavailable, the pharmacist will contact the prescriber to discuss use of antihemophilic factor(Helixate FS) as an alternative.
infliximab in Severe Chemotherapy Induced Colitis Restrictions Updated Infliximab approved uses expanded to include treatment of steroid refractory immunotherapy-induced severe colitis.
Tezacaftor-ivacaftor and ivacaftor (Symdeko) Non-Formulary Inpatients may use own supply.
Acetaminophen /Codeine 15 mg (Tylenol #2)

Reclassified as Non-Formulary

Automatic Interchange Approved
Tylenol #2 was reclassified due to rare use.  Orders for Tylenol #2 will be interchanged to the liquid preparation of acetaminophen (120 mg) +codeine (12 mg)/  5 mL to provide a similar dosage of codeine. Tylenol #3 remains a formulary agent.
Nitrous Oxide in Labor/Delivery Patient Care Guidelines and Protocol Approved Implementation of nitrous oxide for analgesia in labor and delivery pending further approval from other institutional committees.
Patient’s Own Supply of Medication and Patient Self-Administration Policy Patient Care Guidelines Update Approved Criteria for use of patient’s own medications while in the hospital were updated.  If a prescriber specifies patient’s own supply, but the medication does not meet criteria, the Committee approved an automatic EMR adjustment to use of the hospital’s supply of the medication (i.e. pharmacy dispense).
pegfilgrastim (Neulasta) Oupatient Use Medication Use Evaluation & Criteria for Use Reviewed Criteria for use restrictions approved.
