P & T Update Memo May 2018

Following is a P and T Committee update (from the May 22nd meeting). Starting date for specific programs is 1 June 2018, unless otherwise noted. Please let me know if you have any questions or comments. Don’t forget to check the “New Drug Monitoring Spotlight” section of the website, which provides important monitoring information about newly approved drugs. Also, attached is the updated version of the “Not Stocked, Not Ordered” drug list with the new additions highlighted in red.

minocycline (Ximino)

This is a tetracycline related acne product and is interchanged to regular minocycline generic in similar total daily dosages.
Maggot larval therapy (BioBag by BioMonde) Formulary Because of several advantages, the BioMonde maggot product was approved to take the place of the Monarch product on the formulary.
Conivaptan (Vaprisol) Changed from Formulary, Restricted to Non-Formulary. Reclassified as non-formulary due to no utilization resulting in wasted stock. Alternative therapies include tolvaptan and 3% sodium chloride.
edrophonium (Enlon) Reclassified as Non-Formulary Enlon is no longer available on market. Similar drugs are neostigmine and pyridostigmine.
Prenatal Vitamin Formulary Update Workhorse Formulation Change The stocked prenatal vitamin serving as the formulary workhorse will be changed to a chewable tablet formulation.
2017 Antibiogram Update Reviewed & Approved 2017 Antibiogram will be available on the Huntsville Hospital intranet.
