P & T Update Memo November 2017

Following is a P and T Committee update (from the November 28th meeting). Starting date for specific programs is 4 December 2017, unless otherwise noted. Please let me know if you have any questions or comments. Don’t forget to check the “New Drug Monitoring Spotlight” section of the website, which provides important monitoring information about newly approved drugs. Also, attached is the updated version of the “Not Stocked, Not Ordered” drug list with the new additions highlighted in red.

lixisenatide (Adlyxin) Non-Formulary

Initiation should be deferred until after hospital discharge. Inpatients may use their own supply. If a GLP-1 agonist is required, liraglutide is an alternative currently on formulary.

insulin glargine-lixisenatide (Soliqua) Non-Formulary Soliqua is a fixed dose of insulin glargine and lixisenatide. Initiation should be deferred until after hospital discharge. Inpatients may use their own supply.
insulin degludec-liraglutide (Xultophy) Non-Formulary Xultophy is a fixed dose of insulin degludec and liraglutide. Initiation should be deferred until after hospital discharge. Inpatients may use their own supply.
guselkumab (Tremfya) Non-Formulary Initiation should be deferred until after hospital discharge. Inpatients may use their own supply.
dupilumab (Dupixent) Non-Formulary Initiation should be deferred until after hospital discharge. Inpatients may use their own supply.
edaravone (Radicava) Non-Formulary Patient candidate pool is small and
clinical benefits are limited. Current
literature does not support use of this
agent.
gadobenate dimeglumine (Multihance) Formulary, Restricted Restricted to use by radiology in
selected cardiac procedures.
atropine-pralidoxime (DuoDote) Reclassified as Formulary, Restricted Temporary formulary status for the
shelf-life of the stock received at no
cost to the facility (2-3 years). After
current stock expires, DuoDote will
return to Non-Formulary status with a
the individual components dispensed
separately.
After Hours Remote Order Verification -- Madison Hospital Non-Formulary Between the hours of 22:30-06:30 on
Monday-Friday, and 20:00-0800 on
Saturday and Sunday, pharmacists at
Huntsville Hospital Main will provide
verification and telephone support
pharmaceutical services for patients at
