***ON CALL*** Give vancomycin 1 gm for weight less than 80 kg (176 lbs) or 1.5 gm for weight 80 kg or more. Give in Holding Room.
Per policy, orders for these non-formulary agents will be changed to formulary equivalents.

vancomycin oral any strength/frequency vancomycin oral 125 mg q6h x 10 days. If 500 mg ordered, evaluate whether appropriate per protocol for fulminant infection (hypotension, shock, ileus, or megacolon). If these criteria not met, change to 125 mg po q6h x 10 days.
