Medication comments:

Available for acute ischemic stroke and STEMI patients. Multiple kits for Stroke will be stocked in the Omnicell cabinets in the ED. Tenecteplase for STEMI to be stocked in Pharmacy. If Tenecteplase for an inpatient is needed – pharmacy to prepare.
tenecteplase dosing for acute ischemic stroke
Dose: 0.25 mg/kg
• Use actual body weight to calculate dose.
• Draw up dose in 5 mL syringe.
• Maximum dose: 25 mg
*Precipitation may occur when tenecteplase is administered in an IV line containing dextrose.

 • Flush IV line with 10 mL Normal Saline before and after administration.
 • Administer dose IV push over 5 seconds.

 • Physician/Pharmacist independently verify calculation of dose prior to administration.
 • Two RNs independently verify dosage and pump settings prior to administration.
Per policy, these medications require cardiac and/or special monitoring.
