Per policy, order is limited to 8 x 125 mg doses (1 gram elemental iron total) if not otherwise specified by the prescriber.

(250 - 1000 ml NS)
ferric carboxymaltose (Injectafer) 50 mg   • < 50 kg: 15 mg/kg elemental iron on day 1; repeat dose after at least 7 days (maximum: 1500 mg elemental iron per course). May repeat course of therapy if anemia reoccurs.
• ≥ 50 kg: 750 mg on day 1; repeat dose after at least 7 days (maximum: 1500 mg per course). May repeat course if anemia reoccurs.
ferumoxytol (Feraheme) 30 mg 510 mg x 2 doses (3 to 8 days apart) 510 mg x 2 doses (3 to 8 days apart)
iron sucrose (Venofer) 20 mg 100 mg IV 3 times/week
(every dialysis) x 10 doses
200 mg IV x 5 doses during a 14 day period.
Alternatively:
 (a) 500mg slow IV infusion on day 1 and day 14.
OR
 (b) 300 mg, 300 mg, then 400 mg - each dose 14 days apart.
