Any new GLP-1 agonist (Victoza, Ozempic, Wegovy, Trulicity) orders from a patient's home med use will be held for a minimum of 7 days from date of admission. This process applies to IMH and IDBH but does not apply to HBSNF patients. When an order is received, it is to be discontinued per protocol. From there the pharmacist needs to put in a quick intervention on Quantifi to follow up at day 6 of admission to discuss with the provider if want patient to bring from home or can use an alternative agent while the patient is admitted.
