Medication comments:

Albuterol/ipratropium (DuoNeb) nebs is our formulary short-acting Beta-agonist/anticholinergic combination agent. Orders for this type of metered dose inhalers will be converted to DuoNeb.
Per policy, use of these medications is restricted as noted below:

Inpatient/SNF orders will be converted to albuterol-ipratropium (DuoNeb) nebs.
Per policy, orders for these non-formulary agents will be changed to formulary equivalents.

tiotropium (Spiriva Respimat) 2 INH daily
