Key points:

  • Orders for sotalol require a pharmacy consult
  • Pharmacists must assess for contraindications to therapy
  • For new starts, pharmacists must assess appropriateness of dose based on renal function and may order baseline labs and EKG "per policy"
  • Pharmacists will monitor potassium and magnesium and order supplementation "per policy"
  • Pharmacists will document in Epic using i-Vents and consult notes at least daily
  • Pharmacists must provide patient education for all patients on therapy
