System_RXCLIN 127 Pharmacy Procedure for Oseltamivir Alert (Vigilanz)

Key points:

  • Based on microbiological data, pharmacists will evaluate indication, dosing, and frequency
  • If consulted, pharmacist will optimize dosing “per policy” or will recommend discontinuation of therapy. If not consulted, pharmacist will contact MD for therapy optiminization
  • The recommended frequency and duration for influenza treatment is twice daily for 5 days
  • The recommended frequency and duration for influenza prophylaxis is once daily for 10 days
  • Dose must be renally adjusted
  • Ensure patient is on appropriate isolation status. Nursing may change isolation orders "per policy"
