Consults, protocols, and therapeutic interchanges

Key points:

• Pharmacists will evaluate indication and current patient status and will optimize dosing and monitor labs “per policy”
• All orders for ibutilide must be treated as a STAT order
• Pharmacists will verify baseline potassium, magnesium, and QTc
• Pharmacy will send one magnesium bag, and 2 grams of magnesium is to be administered before ibutilide
• Pharmacists may close i-Vent after administration of drug
