Fosaprepitant is approved on HM formulary for:
1) nausea and vomiting associated with initial and repeat courses of moderate to highly emetogenic chemotherapy
2) use in patients undergoing bariatric/foregut surgery that are unable to fill an outpatient prescription for aprepitant prior to surgery
