Key Points:

  • Orders for conivaptan require a pharmacy consult
  • Pharmacists will evaluate indication and current patient status and will contact physician to change conivaptan orders; orders cannot be changed “per policy”
  • Pharmacists will document in Epic using i-Vents and consult notes at least daily
