Key Points:

  • Due to risk of agranulocytosis associated with Clozaril (clozapine) therapy, ALL patients receiving clozapine therapy must be monitored and registered with the manufacturer's specific surveillance program
  • Pharmacist verifying order is responsible for ensuring patient is elibible and registered with TCPR before dispensing clozapine

  • Pharmacist must document "eligibility code/patient ID" from TCPR in patient's chart in EPIC
  • Pharmacist may add patient to The Methodist Hospital's TCPR patient list
