System_RXCLIN 143 Pharmacy Procedure for IV-PO Alert (Vigilanz)

Key points:

  • Vigilanz will alert pharmacist to the presence of IV antimicrobials that are 100% bioavailable on a patient's active medication profile for >48 hours
  • Pharmacist will review the patient's profile for appropriateness of intervention and contact the appropriate provider if the patient is clinically stable and can tolerate oral intake as detailed in policy
  • Pharmacist must document intervention
