System_RXCLIN 102 Pharmacy Consult for Argatroban for Heparin Induced Thrombocytopenia

Key Points:

  • Pharmacists will evaluate indication and current patient status and will optimize dosing and monitor labs “per policy”
  • Pharmacist will order PTT monitoring 3 hours after initiation and rate changes
  • Document a heparin/low molecular weight heparin allergy if heparin-induced thrombocytopenia is confirmed
  • Pharmacists will document in Epic using i-Vents and consult notes with every rate change and at least daily
