System_RXCLIN 138 Pharmacy Procedure for Thymoglobulin

Key points:

  • Pharmacist must verify the following:
    • Indication
    • WBC and platelet count
    • Central line access
    • Whether premedications and/or rescue medications have been ordered
  • Thymoglobulin must be infused through a 0.22-micron filter into a central line
  • Thymoglobulin expires after 24 hours
  • Pharmacist must contact nurse to determine appropriate administration time
    • If normal business hours, contact transplant pharmacist
  • Pharmacist must call SPA pharmacist (1-2759) and let them know when medication should be prepared and sent to floor
  • Dose will be automatically be rounded to the nearest 25 mg per policy
  • For all doses being administered in the operating room, the operating room nurse will contact the operating room pharmacist (1-0670) to coordinate preparation and delivery
  • For all doses NOT being administered in the operating room, the clinical pharmacist must call the SPA pharmacist (1-4306) to coordinate preparation and delivery
