Ambien CR is non-formulary, not stocked. Interchange to regular zolpidem. Lunesta can be used as a back-up drug (non-benzodiazepine with a longer half-life) if needed.
Due to new FDA dosing recommendations, P&T approved a dose adjustment from zolpidem 10 mg to 5 mg for female patients, if the medication was started in the hospital. The dose should not be adjusted for female patients that have taken 10 mg as a chronic outpatient medication. Reviewed: April 23, 2013
