  • 600 mg tablet oral QID (Patients  12-14 years old)
  • 600 mg tablet oral QID (Patients 15 years old or greater)

montelukast (Singulair)

  • 5 mg tablet chewable oral Daily                                                                                         
  • 10 mg tablet oral Daily                                           

zileuton (Zyflo)

  • 600 mg extended release tablet oral 1200 mg BID (Patients 12-14 years old)
  • 600 mg extended release tablet oral 1200 mg BID (Patients 15 years old or greater)

 

montelukast (Singulair)

  • 5 mg tablet chewable oral Daily                                
  • 10 mg tablet oral Daily 

Convert Zyflo and Zyflo CR to montelukast unless 'no sub' is written. If Zyflo CR is written 'no sub' interchange to immediate release zileuton in an equivalent dose (i.e., Zyflo CR 1200 mg BID should be coverted to Zyflo 600 mg QID).
