Comments:

 Interchange Voquezna Dual Pak to: amoxicillin 1000 mg PO TID and pantoprazole 40 mg PO BID X 14 days

 

Evaluate for clarithromycin resistant H. pylori prior to automatically interchanging (pharmacist to check laboratory results or contact outpatient physician/outside facility for patients being admitted). If resistance is present, the patient may use their own supply of Voquzna Dual Pak or the pharmacist should recommend guideline-directed quadruple therapy to physician.
