Trelstar was approved as a formulary agent, but the prescriber should be strongly encouraged to initiate and administer the drug in the outpatient setting where the reimbursement is better.

If necessary to administer to an inpatient, only the 3.75 mg dose (which covers about 4 weeks) should be given. It will not be generally stocked, but a dose can be ordered in if needed.

Triptodur is non-formulary. Defer initiation to after discharge.
