Qudexy XR and Trokendi XR are classified as non-formulary, not stocked. Interchange to immediate release topiramate at the same total daily dose split into 2 doses (BID) in adults.

Pediatrics - Do not automatically interchange these drugs for pediatric patients. Patients may use their own supply of QUDEXY XR or TROKENDI XR if needed; contact phsyician if home supply unavailable.


Topiramate Oral Solution: Restricted for use in patients who are unable to take topiramate sprinkle capsules or tablets. Not routinely stocked.
