Formulary, but not routinely stocked, restricted to Oncology. Unusual inpatient use is expected. If a case arises, it will be evaluated on a individual case basis. If possible, the drug should be initiated in the outpatient setting, and if needed for an inpatient, attempt to aquire through the established outpatient insurance mechanisms.
