Comments:

Envarsus XR should be interchanged to tacrolimus immediate release on a 1 mg: 1.25-1.42 mg ratio based on total daily dose (TDD) then rounded to the nearest dosage form(s) available in divided doses.

  • Envarsus XR 1 mg daily ---> Prograf 1.25 mg - 1.42 mg TDD ---> Prograf 0.5 mg TID
  • Envarsus XR 4 mg daily ---> Prograf 5 mg - 5.71 mg TDD ---> Prograf 3 mg qAM + 2 mg qPM
