Fleet Phosphasoda NO SUBSTITUTION Orders: Restricted to patients with adequate renal function (CrCl > 30 mL/min). One dose (118 mL) may be administered per 24 hours, not to exceed a total of 2 doses in a 14-day period. Patients with CrCl < 30 mL/min, ESRD, or requiring renal replacement therapy should continue to be interchanged to Fleet Mineral Oil Enemas.

Give 32 OsmoPrep tablets for 32-40 Visicol tablets. If less than 32 Visicol tablets are ordered, convert Visicol to OsmoPrep on a 1:1 basis.
