Elitek ADULT Criteria for Use (In emergent, after-hours cases, rasburicase may be ordered by a non-oncology provider given the criteria for use is met).

 

Adults: Use restricted to oncology prescribers for treatment of hyperuricemia in patients who meet approved criteria set. Use criteria set for patient selection and protocol order set for adult patients.

Adult Dose Capping: Initial dose is capped at 3 mg for patients weight <90 kg and baseline uric acid level <18 mg/dL

 

 


Pediatrics: Restricted to use by oncology prescribers.

PEDIATRIC Rasburicase Criteria for Use

Pediatric Dose Rounding:

  • < 10 kg:   0.2 mg/kg
  • 10 – 16 kg: 1.5 mg
  • 16.1 – 28 kg: 3 mg
  • 28.1 – 45 kg: 4.5 mg
  • 45.1 – 60 kg: 6 mg
  • ≥ 60.1 kg: 7.5 mg
