Vitamin K injection -- Discourage IV administration. If necessary, give IV based on the following P&T approved policy: For adult patients: For doses of 10 mg or less, the drug should be diluted in 50 mL D5W or NS and administered over 30 minutes. For doses over 10 mg, the drug should be diluted in 100 mL D5W or NS and administed over 1 hour. Doses over 60 mg should be given at a rate of 1 mg/minute. If an urgent situation exists where vitamin K needs to be administered (ie, urgent cardiac catherization), Vitamin K may be administered at a rate of no faster than 1 mg/minute. For pediatric patients: Dilute doses 10 mg or less to a concentration of 0.2 mg/mL and give over 30 minutes. Doses >10 mg are diluted in 100 mL and given over 1 hour. In urgent situations, the dose may be diluted to 10 mL with NS and given over 15-30 minutes. Rate not to exceed 1 mg/minute.
