phenoxybenzamine (Dibenzyline)

  • Initial -- 10 mg PO Qday

  • 80 mg PO BID
  • Max -- 80 mg PO TID

doxazosin (Cardura)

  • Initial: 1 mg PO Qday
  • 1 mg PO BID
  • 2 mg PO TID (or 4 mg Qday)
  • 2 mg PO TID (or 6 mg Qday)
  • 4 mg PO BID (or 8 mg Qday)
  • 4 mg PO TID (or 12 mg Qday)
  • Max: 8 mg PO BID (or 16 mg Qday)

Maintenance of Phenoxybenzamine is 20 – 40 mg (2-3 times/day): 40 mg to 120 mg/day

Maintenance of doxazosin is 4 – 8 mg/day (may be divided doses)


For pheochromocytoma cases, if phenoxybenzamine is ordered interchange to doxazosin.  This situation is rare, and the cost of phenoxybenzamine has drastically increased.  Doxazosin is the most reasonable alternative among other alpha blockers.  See formulary document for references.
