Formulary status, restricted to ID physician use, use limited to patients with influenza who are not a candidate for enteral oseltamivir (resistance or intolerance), cannot take inhaled zanamivir, and there are no suitable alternatives to intravenous therapy (i.e. cannot do enteral administration due to malabsorption, GI obstruction, etc.)
