Comments:

Penicillin G potassium is the workhorse IV penicillin product.

Penicillin G sodium should be automatically interchanged to penicillin G potassium on a 1:1 dosage ratio, unless the practitioner believes that the extra potassium cannot be tolerated. In these cases penicillin G sodium may be used. Pharmacists may perform this interchange automatically.

Penicillin G infusions cannot be auto-programmed due to pump limitations. These infusions must be manually programmed. When penicillin G is selected from the pump library, a message will be displayed stating that 1 unit in the pump = 1,000 units of penicillin G. The ordered dose divided by 1,000 should be entered into the drug amount field

Automatic renal adjustment per guidelines. See Antimicrobial Dosing Guideline and Renal Adjustment Policy (Adult) for more information.  CrCl calculation policy available here.
