Comments:

Invega Trinza and Invega Hafyera are similar to Invega Sustenna, but are administered once every 3 months and once every 6 months, respectively. Therapy should be deferred to the outpatient setting.

Invega Sustenna is classified as formulary for the loading doses (234 mg and 156 mg), which are reimbursed through a drug company voucher program.

RECORD all doses dispensed in the Invega Sustenna database (located on the Pharmacy Shared Drive- S:\Access\CentralPharm\Sample Drug Dispensing Log).

Before Verifying Invega Sustenna Loading Dose:

1) Confirm prescriber is a psychiatrist or has consulted a psychiatrist.

(ER Physicians cannot prescribe Invega Sustenna through the free Loading Dose Program offered through Janssen Pharmaceuticals unless a psychiatrist MD has been consulted and recommended to use the drug. If the psychiatrist is not consulted and/or this is not documented, we cannot send in for the free drug replacement. When verifying Invega Sustenna order by a non-psychiatrist MD, contact the prescriber to assure that a psychiatrist has been consulted, and that this is documented in the appropriate section of the medical record.)

MDs currently registered to prescriber Invega Sustenna Loading Dose: Quratulain Agha, Venkata Devabhaktuni, Tarak Vasavada, Amjed Javaid, Praveen Jetty, Scariya Kumar, Trevor Lindsay, Senthil Rajaram Manoharam, Donna Scott, Patrick Tapia

2) Check Invega Sustenna Database (located on the Pharmacy Shared Drive- S:\Access\CentralPharm) - patients are allowed 2 free doses in a calendar year (i.e., Jan~Dec).

Both strengths (234 mg or 156 mg) count toward the 2 total free doses

3) If the patient has received 2 doses in the current calendar year, contact the prescriber to determine if the dose can deferred until after discharge.

If prescriber insists it is necessary to give the dose as an inpatient, the dose may be dispensed. However, it will not be a free dose.

The maintenance dose (117 mg) is Non-Formulary. The INVEGA SUSTENNA 117 mg strength is available in Cerner (even though we do NOT stock it) so that this dose can be entered into Cerner to create a complete Med Rec Record upon the patient's discharge. Please do NOT remove the 117 mg dose from the patient's record (unless it is discontinued by the MD).
