Adults: Interchange at 1:1 ratio.

Pediatric: Convert using dose appropriate for indication and weight; not required to contact MD, but may be necessary to determine indication for use.

Interchange 250 mg dicloxacillin for 500 mg of oral oxacillin, or proportionally.
