Comments:

Defer initiation to the outpatient setting. Inpatient may use their own supply if available and requested, otherwise, orders will be automatically interchanged to the following regimen:

  • pantoprazole 40 mg three times daily
  • amoxicillin 1,000 mg three times daily
  • rifabutin 150 mg once daily
