Use 200 mg IR dose for doses of 150 - 400 mg IR and 100 mg tab for doses less than 150 mg. For ER doses, divide daily dose of IR in bid-tid dosing regimen of IR tablets (to similar dose that can be achieved with formulary strengths - pharmacist OTC dose rounding). For doses over 1,000 mg /day, use 500 mg tablets and round off to nearest 500 mg dose. For doses greater than 1,000 mg, give IR tablets bid/tid.


When interchanging niacinamide, do not interchange immediate release products with sustained release products or vice versa. This can increase chances of adverse drug reactions.
