As of March 2022, DEKAs Plus took the place of AquADEK as the formulary product.  ADEKs & AquADEKs have been manufacturer discontinued.

DEKAs Essential contains only vitamins A, D, E, & K.

 


Tandem-Plus, Tandem-OB and Tandem-F are non-formulary/not to stock or order. Automatically interchange Tandem-Plus, Tandem-OB, and Tandem-F (one tablet daily) to Iberet Folic 500 (one tablet daily).

Centrum Ultra Silver (for Men, for Women) are classified as NON-FORMULARY, NOT STOCKED OR ORDERED. A formulary generic multivitamin should be used instead.
