Comments:

All once daily orders for metoprolol (without a salt form specified) will be converted to metoprolol succinate (ToprOL XL) to avoid large once daily doses of the immediate release salt form.


Kapspargo is non-formulary.

For adults who cannot take tablets, automatic interchange to crushed metropolol tartrate tablets. If interchanging long acting succinate once daily to crushed tartrate tablets, BID at the same total dosage. If adult patient can tolerate tablets, convert Kapspargo to metoprolol succinate (ToprOL XL) tablets at same total daily dose.

For pediatrics orders of Kapspargo: if pediatric patient can tolerate tablets and extended release product perferred, extended release metoprolol is available. Contact prescriber prior to changing metoprolol product or dosage.
