Daytrana (methylphenidate patch) is approved for formulary if a patient already on the patch needs to continue therapy in the hospital and doesn't have their own. Initiation should be deferred to the outpatient setting, and if possible the patient should use their home medication for inpatient use. If this is not possible, a small number of patches may be ordered.
QUILLAVANT XR is classified as non-formulary, not stocked. Contact the prescriber to switch to an alternative methylphenidate product; or if necessary, the patient may use his/her own supply during admission.
