metFORMIN -- hold for 48 hours post IV contrast media per radiologist, contraindications with CHF, renal disease, hepatic disease, and age. Pharmacist may order SCr.
Interchange to individual ingredients dispensed separately. For Janumet and Zituvimet, use immediate release metFORMIN as the metFORMIN component. For Janumet XR and Zituvimet XR, use metFORMIN XR as the metFORMIN component.
