Comments:

Probiotics are restricted to use by GI and pediatric providers. Since probiotics are considered a dietary supplement, if they are ordered by a non-GI/Peds provider, the verifying pharmacist is authorized to discontinue the order.

 

Culturelle Kids serves as formulary workhorse for pediatric probiotics. Other pediatric formulations of probiotics should be interchanged to Culturelle Kids at an appropriate dosage for the patient.

Pediatric Automatic Discontinuation:  After notifying the ordering provider, pharmacists may automatically discontinue probiotic orders for pediatric patients at HHWC once confirmation of central venous catheter placement.
