Dialysis Outpatients (unattached patients): Venofer preferred.

OPM (Non-Dialysis Patient): Feraheme is the preferred agent in non-dialysis outpatients based on reimbursement issues with Venofer.  Contact MD to discuss Feraheme as the preferred alternative in this situation.

 

 

Outpatient Medical (Adults): May interchange to the most cost effective intravenous iron product at an equivalent elemental dose. The prescriber should be notified of any interchange.
