  • 1.25 mg-500 mg tablet                                                         
  • 2.5 mg-500 mg tablet                                                              
  • 5 mg-500 mg tablet oral

glipizide (Glucotrol) AND metFORMIN (Glucophage)

  • 2.5 mg glipiZIDE tablet PLUS ONE 500 mg metFORMIN tablet 
  • 5 mg glipiZIDE tablet PLUS ONE 500 mg metFORMIN tablet
  • 10 mg glipiZIDE tablet PLUS ONE 500 mg metFORMIN tablet

 

Non-formulary.  Interchange to individual components. If ordered as no substitution, dispense glyburide and metformin as separate products.

metFORMIN -- hold for 48 hours post IV contrast media per radiologist, contraindications with CHF, renal disease, hepatic disease, and age. Pharmacist may order SCr.
