Gralise is classified as non-formulary, not stocked for inpatient use. Interchange to an equivalent regimen of immediate release gabapentin, given BID, TID, or QID, depending on the dosage. Horizant is also classified as non-formulary, not stocked. Interchange to equivalent regimen of immediate release gabapentin.

Gralise may be used in OUTPATIENT CLINICS.
